How do I set up an account?

CH-F will provide you with an invitation via email (if you provided an email address) or a printed copy of instructions for accessing the MyCareCorner website. Once you have completed the initial setup of your account, you will only need your unique username and password to sign into the Patient Portal in the future.

How do I access the Patient Portal once I have completed the account setup process?

For future visits to the Patient Portal, visit the login page at https://mycarecorner.net/

Will I receive emails after every admission to the hospital or clinic visit?

No. After each visit, a new summary of care document will automatically post to your Patient Portal account. You may access the document any time after you are discharged. Once the initial invitation email has been sent and your account has been created, you will not receive emails after each visit.

Who else can see my personal health information or access my Patient Portal?

You, anyone you provide your User ID and Password to, and anyone you designate as your Patient Portal Authorized Representative will have access to your Clinical Information. Your Patient Portal Authorized Representative will only have access to your Patient Portal.

Can a child accompany a patient to an appointment?

For all upcoming appointments, patients may come with one family member. No children will be allowed to accompany patients to their appointment. Please contact the office with any further questions.
