What is the patient portal login?

Type ConnectEHR in the search box. If the search results do not display automatically, click the magnifying glass. Click + Cert ID for ConnectEHR 3.0. Add the quality measure portal module to your cart: Type CQMSolution in the search box. If the search results do not display automatically, click the magnifying glass.

How do patients log in to their EHR portal?

When a patient portal is enabled, Eyefinity EHR assigns a username to the patient and sends the username and login instructions to the email address listed in the patient's record. When the patient logs in for the first time, they confirm their identity by entering their date of birth and last name, and then they create a password.
