Please Note: In order for a minor patient under the age of 13 or a parent/caregiver of a minor patient under the age of 13 to enroll in the FollowMyHealth ® patient portal for Quillen ETSU Physicians, a consent form must be signed and an invitation sent. The consent form can be dropped off at your provider's office or faxed to (423) 433-6060.
