How do I cancel or reschedule my appointment?

If you need to cancel or reschedule your appointment, we ask that you call your provider’s office to let them know in advance. Your provider may have additional guidelines regarding cancellation. Please find your provider’s office here.

What should I bring to my visit?

If you have insurance, please bring a copy of your insurance card to each visit. We will submit the claim to the insurance company on your behalf, but please be prepared to pay your co-pay and any deductible on the day of your visit.
