The University of Vermont Health Network - Elizabethtown Community Hospital (ECH) has a network of community health centers in Essex County. These health centers are run by dedicated care teams that bring a wealth of experience and connection to all that the University of Vermont (UVM) Health Network has to offer.
This interactive web portal empowers all Community Hospital patients and their families to take a more active role in their care by providing easy, secure access to health information and online communication with Patient Portal staff. All of the information in the Patient Portal comes from your Community Hospital Electronic Health Record.
Children 12 and older may set up access to their MyHealth Patient Portal by clicking on the “I do not have an account” button above. To comply with Federal and Colorado State Laws regarding privacy and minors, Community Hospital has chosen to limit access to patient portals for minors over 12 years of age.
Elizabethtown Community Health Center provides preventive services, diagnosis, and treatment to patients of all ages. Our care teams take care of a wide range of conditions across the lifespan. In the event that our clinic staff cannot treat a certain condition at a community health center, they will provide referrals to services within the ECH and UVM Health Network.

ECH has six community health centers located throughout Essex County. These health centers benefit from their connection to ECH and the UVM Health Network, including expertise and referrals to our full range of health services. Our health centers are staffed by caring medical professionals that offer essential primary care, urgent care, and some specialist services to patients of all ages. By being closer to the community, we can provide our patients with a high level of compassionate care closer to home.
Health and wellness maintenance and preventive screenings for children, adults, and older patients, including routine exams, pap smears, or physicals for work, schools, camps, or sports activities. Diagnosis and management of common health conditions such as high blood pressure, diabetes, and heart disease.
Health Center billing is handled by Visualutions. If you have payment concerns, the Hospital Business Office at ECH may be able to help in person or by phone at (518) 873-3139.
