If you are unable to keep a scheduled appointment, please contact our office by phone to cancel as soon as possible to avoid a no-show charge.

Initial Visit

If this is your first visit to Elite Medical please bring a Photo ID, insurance Cards, and all medication you are currently taking including any over the counter medications and/or supplements. For your convenience, new patient registration forms may be downloaded below. Please bring completed forms to your first visit.

Insurances Accepted and Fees

We accept most major insurances including but not limited to: Acclaim, Aetna, Blue Cross Blue Shield, Cigna, Medicare, Medicad, MPCN, MSCAN, Tricare, and United Healthcare. If you have questions regarding insurance coverage please contact our office at 662-205-4652.

Medical Records

We will be happy to assist in the event you need a copy of your medical records or records need to be sent to another provider or entity. All requests should be made in writing and appropriate consent may be required depending on the nature of the request. Also a medical record fee may be charged for certain request.

How many patients are in a concierge practice?

What makes concierge medicine more attractive? A traditional private practice consists of 2500-3500 patients within that practice at any time. While a concierge model limits the practice to 300-500 patients.

Is it better to have a high deductible or low premium insurance?

It is best to pair your membership with a high deductible, lower premium insurance plan. Due to the high cost of hospitalizations and emergency care you need to be protected in case of catastrophic events such as trauma, cancer or serious medical events.
