How do I make sure I am receiving patient portal emails?

MyChart/Patient Portal In partnership with OCHIN, who supports our clinic’s electronic health record system, we are offering our patients secure, online access to their personal medical charts. Using OCHIN MyChart, patients can see lab results, view medication lists, make appointments, and much more—online, any time of the day or night.

Patient Health Portal

Through the convenient and secure Patient Health Portal, Imperial Valley Family Care has made it easier for you to communicate with your physician. Patient Portal is available to you in the comfort of your home, office, or on the road.

Patient Forms

To assist our patients with their office visits we now offer our patient information and medical history forms available for download in English and Spanish. Please select your required language and download. On the day of your appointment please bring in your completed forms, insurance cards, copays and/or deductibles.

How to Choose Your Username and Password

Create Your USERNAME, which must be between 6-50 characters and is case sensitive.

Using the Patient Portal

From your welcome screen, you can see and access your inbox, upcoming appointments, lab results, medications, and more.

Notifications

You will receive email notices when new information is available for you on the Patient Portal. No personal health information will be sent to your email account, only notifications from the Patient Portal.
