MyChart Patient Portal. MyChart is a new patient portal that keeps all of your health information together in one place. Bill Pay and CareLink Re-Enrollment will continue to serve you. Items that are part of your medical record include prescriptions, lab results, allergies and immunizations. These will transfer when you enroll in MyChart.
Federal law prohibits UHS from including the television and telephone charges in your daily room rate. This service is provided by Pathway Solutions, LLC 3840 East Robinson Rd, Suite 140, Amherst, NY 14228. Pathway’s telephone number is 888.891.6523. Costs for long-distance calls can’t be charged to your room.

Social workers provide psychological and social assessments and counseling, and coordinate referrals for post-discharge follow-up, nursing home placement, home care nursing, delivery of home medical equipment and end of life care. Whether you need home care services, equipment or supplies, UHS can help.
If you’re admitted to the hospital by your physician, you’ll receive a phone call to let you know when the hospital can admit you. If you’re admitted directly from your physician’s office, please report to Admissions unless your physician directs you otherwise.
Valuables and Personal Property: The hospital can’t assume responsibility for valuables kept in your room or lost or broken dentures, eyeglasses, hearing aids, etc., unless the hospital is clearly responsible for the damage. Meals: All UHS Hospitals provide freshly prepared, nutritious meals for patients. A large variety of items are available.
When you come to the hospital, bring a list of the medications, nutritional supplements, vitamins, over-the-counter medications and herbal medications you are taking at home. If you bring your medications to the hospital, send them home with your family once the nursing staff has seen them.
