Since 2014, the East Cliff Family Health Center serves the primary health care needs of men, women and children regardless of economic status, through high-quality, patient-centered programs that offer primary care, pediatric services, mental health education, health coverage enrollment, food access programs and more.
Make sure that you have previously logged into your Patient Portal account. On the Welcome to healow screen, search by your Clinic location by clicking in the white field next to the magnifying glass and typing the practice name (Santa Cruz Women’s Health Center or East Cliff Family Health Center).
Our Patient Portal uses the latest security measures to ensure your health information is secure and we will not give your email address to companies or individuals outside of East Jordan Family Health Center. You will need to get an enrollment token from the Health Center.

It can take around 3 business days after blood has been collected for the results to be sent to your medical provider. Once they receive the results, the labs will be published online when the provider has signed off on the labs.
Radiology reports do not load to the portal. Rather your PCP will document their own results which you will be able to access.
The minimum amount of time to request an appointment is 3 days, but does not guarantee an appointment. As schedules allow, we will do our best to provide an appointment in a reasonable amount of time. For all urgent requests, it is best to call the office at (831) 427-3500.
Currently we are not refilling prescriptions from the patient portal. Please contact your pharmacy to request medication refills.

How to Enroll in the Patient Portal: 1 You will need to get an enrollment token from the Health Center. You can either call or stop into the Health Center and ask for enrollment token. Your enrollment token will be a series of numbers. 2 Click on the Patient Portal Login box above. 3 Enter your enrollment token that you were given from your Health Center and enter your email address. You will also need to enter your birth date.#N#The email address you enter must be the one you provided to the Health Center when you received your token.#N#If you do not have an email address, assistance is available. 4 Choose a username and password.#N#Your username and password are case sensitive.#N#Usernames must be 6-20 characters.#N#Passwords must be 6-20 characters and contain at least one (1) number. 5 Choose a login security question from the list provided and provide your answer for the question.#N#The question you select will need to be answered every time you log into your Portal account, due to your account containing confidential medical information that must be protected. 6 Set up your accounts secure password reset feature.#N#The Patient Portal will ask you to select a question and provide an answer.#N#If you forget your password, the Portal will be able to reset your password by answering the selected question.
Once logged in to the Patient Portal, choose ‘Schedule’ on the upper right menu bar. Then choose ‘Request Appointments’. You can then choose ‘Appointment Request’ and select your Health Center. You will then need to select your provider.
Why activate your patient portal account? A patient portal offers several benefits. You can bypass your practice’s call center to manage appointments, request prescriptions, or directly communicate to your healthcare team. You can also view your health records, access visit summaries and other documentation.
Choose a username and password. Your username and password are case sensitive. Usernames must be 6-20 characters. Passwords must be 6-20 characters and contain at least one (1) number.
You can invite trusted representatives to access your patient portal in your “Profile.”. If they do not have a patient portal account, they will be prompted to create one. From there, they can toggle between their own account and any accounts that have been shared with them.
These laws cover a teen’s sexual health, including birth control, STDs, and pregnancy. They also cover the care of a teen’s child. East Jordan Family Health Center must follow these laws. Because of this, children between the ages of 12 and 17 will not be signed up for the Patient Portal to protect their privacy.
Please note: The Bellaire and East Jordan Family Health Centers see patients by appointment only. Walk-ins are NOT accepted.
