How do I sign up for my patient connect?

Can I use the patient portal to communicate an emergency?

Information and communication drives the MedConnectHealth Patient Portal, helping increase patient satisfaction while boosting your practice’s efficiency. Our secure, web-based service allows patients to use their computer or tablet to quickly and conveniently access information regarding their care.

Who can request access to their spouse's health information?

Legal guardians may request access to their ward's health information. Husbands and wives may request access to their spouse's health information. Adults may request access to their elderly parent's health information — with the patient's permission of course. Request Proxy Access.

What is clinical note?

Clinical Notes, the newest My Patient Connect feature, provides access to important information about your office visits and hospitalizations. The information available via Clinical Notes can help you remember your treatment plan and partner with your providers to ensure you have the best outcomes.

Is Concord Hospital a separate portal?

The patient portal for Concord Hospital - Laconia and Concord Hospital - Franklin is separate from My Patient Connect. In the future, the two portals will be combined but for now, patients need to log in to separate portals based on where they receive their care.

What is Onduo virtual care?

Our virtual care model, centered around the Onduo app, offers individual learning paths; health coaching based on goals; novel sensing devices; food and exercise tracking; in-app telemedicine visits; medication optimizations; and more.

What does Onduo do?

Members receive a blood pressure cuff, digital and connected coaching, medication adherence counseling, and access to telemedicine via the Onduo app. We work to reach a controlled blood pressure target by building healthy habits and behaviors to help reduce the risk of future health problems.

What is Onduo by Verily?

At Onduo by Verily, we use the science of “digital phenotyping” to identify peoples' needs, characteristics, and drivers. We combine that with real-time data from sensing devices, pharmacy records, app activity, and more*, and recommend best next steps to deliver truly personalized care, flagging high-risk individuals for a telemedicine visit, tackling medication adherence issues, or assigning activities to relieve stress.

Does Onduo offer Fitbit?

Onduo, part of Verily, Alphabet’s life sciences subsidiary, announced that it will offer Fitbit devices and Fitbit Premium as part of the company’s approach to whole person health.
