Access to your child's portal will automatically terminate when the child turn's 18. If your child is 18 or over and would like for their parent (s) to have access to their portal, they may fill out the Proxy Access Form to give their parent (s) access.
In order to get access to your child's My Health Info Patient Portal, you must fill out the Proxy Request Form and bring it to the Pediatric Department, your Family Medicine provider's office, or the Medical Records Department at Grady Memorial Hospital. This form may also be filled out at the appointment. If you are not a parent but you have legal guardianship, you must bring a copy of your legal guardianship papers in order to get access to a child's portal.
