Can you disconnect from Duke MyChart?

You acknowledge that it is your sole responsibility to disconnect your personal technology from Duke data collection services and notify Duke of such disconnection (i) if you choose to do so, (ii) if for any reason you no longer use the personal technology and/or have a MyChart account, (iii) if for any reason you no longer have a patient or other relationship with Duke, or (iv) upon the expiration or revocation of any authorization, whichever is earlier. You also acknowledge that Duke may in its discretion limit, suspend or discontinue the remote monitoring service, Duke MyChart service and/or your MyChart account at any time, without notice to you, and with or without any reason for doing so.
