What is Duke MyChart?

The Duke Health MyChart service ("MyChart") is provided solely for the convenience of patients of Duke University Health System, the Private Diagnostic Clinic, and certain other members of the Duke Health Enterprise, as identified in its Notice of Privacy Practices. Your access to and use of MyChart is subject to and governed by these Duke MyChart Terms and Conditions of Use ("Terms and Conditions" or "Agreement") and applicable law, rules and regulations. Throughout this Agreement, Duke University Health System, the Private Diagnostic Clinic, and other members of the Duke Health Enterprise are collectively referred to as "Duke," "we," or "us." Throughout this Agreement, users of MyChart will be referred to as "you" or "Users".

How old do you have to be to use MyChart?

Competent adults who are (a) parents or legal guardians of unemancipated minors between the ages of 12 and 17, and (b) who have “proxy” access to such unemancipated minor’s MyChart account or MyChart information, may request that a MyChart account be established for access by such minor ("Minor (12-17) Account"). The parent or legal guardian of the unemancipated minor between the ages of 12 and 17 must execute the consent linked here and these Terms and Conditions on behalf of the minor. Upon execution of the consent and these Terms and Conditions by the parent or legal guardian of the unemancipated minor between the ages of 12 and 17, the minor will be provided an activation code for the Minor (12-17) Account. Once the minor activates the Minor (12-17) Account, the Minor (12-17) Account will provide the unemancipated minor between the ages of 12 and 17 access to a personal MyChart account, allowing access to the same types of information that all users access under their personal MyChart account. The types of information that may be accessed are described in the consent linked here . Duke may approve or deny requests for a Minor (12-17) Account in its sole and absolute discretion.

What browsers can I use to access MyChart?

MyChart must be accessed with a Secure Sockets Layer (SSL) compatible web browser, such as Internet Explorer, Chrome, Safari or Firefox (for a list of currently supported web browsers and operating systems, go to FAQ, or see the FAQs on this Site).

How long does it take for MyChart to become effective?

In general, modifications to these Terms and Conditions will not apply retroactively and will become effective no sooner than fourteen days after they are posted. However, modifications addressing new functions on MyChart, or made for legal reasons, will be effective on the date you are notified of them.

Can a minor request a personal MyChart account?

Personal MyChart accounts can be requested by following the instructions in the FAQs on this Site. Only competent adults or legally emancipated minors may request a personal MyChart account. Unemancipated minors may not request and will not be gran ted personal MyChart accounts.
