This is a direct cut and paste from a reply when asking Dr.Trimble via email to complete the VA checklist for physicians for my elderly Mom (who had been an office patient for 20 years) to obtain some financial assistance for assisted living: "If you want me to do this in my "spare" time, please do me the courtesy of filling out as much of it as you can; otherwise this will require an office visit".
