Founded in 1970, we established a tradition of great medical care by emphasizing strong personal relationships between patients and physicians.
Supported by our physicians, we also offer services from certified nurse-midwives for low-risk obstetric patients to consider. Our midwives provide patients with prenatal care and delivery services, while ensuring that an adequate safety net exists should the care of a physician be needed.
Your first visit is approximately 60 minutes and includes a medical history, diet history, physical evaluation, and treatment plan development. You will be seen and evaluated by Dr. Simonds, one of our nurse practitioners or physician assistants. Follow-up visits occur monthly. Medications are refilled at each visit for one month at a time.
The initial visit is $250. Follow-up visits are $85. This does not include the cost of medications. Your medical provider will work with you to determine if the medications that we offer would be helpful and the costs for those medications will be discussed with you.
We require a $50 deposit to schedule and reserve your first appointment. This deposit is applied to the cost of your first visit with us.
To provide the best quality of care to our growing patient population, a $25 charge will be added to your office visit fee if you do not show up to your previously scheduled appointment.

The focus of our practice is to provide your family with complete healthcare services. We offer routine and preventative care services including: physical exams and immunizations, illness and injury treatment, chronic disease management and specialist referrals.
