What is portal in medical records?

The portal contains selected, limited medical information from the patient’s medical record and does not reflect the complete contents of the medical record. The purpose of the portal is to give the patient the ability to view current medications, allergies, and recent laboratories as they arrive in the chart.

How to maintain security of medical information?

As a portal user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential. Once the online registration is completed, an email will be sent to you requesting that you activate the account.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – South Baldwin Regional Medical Center will send a text and / or email to the email address and / or cell phone number you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Emergency Note: There is Always a doctor on call for South Branch Family Practice. Please call the office first, BEFORE you go to any urgent care center. These centers are not covered by most insurances and you will have to pay out of pocket.
