I am new to the portal, how do I set up an account?

TEMPORARY USER NAME & PASSWORD (printed out and given to you at your appointment)

How do I get a temporary user name and password?

When you come to your appointment, if you are not enrolled in the patient portal, you will be given a temporary user name & password on a printout from staff. If you did not get one, please go through the steps ab o ve to create a portal account without a username or password or call 817-267-6290 option 4 and ask for a token.

What is a token and how do I get one?

A token is a unique code you can use to log in or reset your portal account. If your token is not working, you can call us at 817-267-6290, press option 8 and ask for a token. Please allow 15 minutes – 24 hours for your token to be active in the system. Enrollment tokens expire after 30 days.
