Patient satisfaction is one of our greatest goals. The staff recognizes the importance of listening closely to each of our patients and addressing all questions or concerns during office visits. We will strive to return all phone calls and portal messages in a timely manner.

Primary care services at Diversity Family Health begin with a new patient visit—where we will spend about an hour learning more about you, your well-being, address any concerns, and discuss ways to optimize your overall health.

Location Specific Services

When you schedule an appointment, you will receive an in-depth medical questionnaire that will be reviewed during your visit to ensure a comprehensive evaluation of your health status. Most primary care appointments will also include a focused or extensive physical examination.

New Patient Registration

For the convenience of our patients, we have a secure, online patient portal where you can register as a new patient. We encourage all our patients to use this portal when possible to expedite the registration process and to save you from having to arrive much earlier for your scheduled appointment.

Appointments

We see patients by appointment. You may schedule an appointment by calling our office at (813) 518-0881. If you are unable to keep your scheduled appointment, we do ask that you give us 24 hours notice so that we may offer your appointment time to someone else in need.

Contacting the Office

The most efficient way to contact us is via the message feature on the patient portal. This is more secure than using standard email. Using the portal, you can request refills, request an appointment or ask basic questions. For more urgent needs, please call our office directly.

Prescription Refills

Please allow 24 hours for prescription refill requests to be completed. There are several options for requesting refills:

My Penn State Health

A patient will use this portal, if you have been hospitalized at Milton S. Hershey Medical Center or St. Joseph Medical Center. Most patients who attend a Penn State Health Medical Group in Dauphin and Cumberland counties will use this portal. Please call your outpatient clinic to verify.

MyChart

Patients of Holy Spirit Medical Center, or any of their outpatient clinics, will use MyChart as their patient portal. View participating clinics.

IQ Health

Patients who live in Berks County, and who have attended a Penn State Health outpatient clinic in this area, will use this portal. View participating clinics.

Care Space (Andrews Patel)

Patients of Andrews Patel will use Care Space as their patient portal.

Your Health File (All About Children)

Patients of All About Children will use Your Health File as their patient portal.
