The MDH HealtheLife patient portal is available for patients 18 years of age or older. The portal includes selected patient data collected at MDH starting June 6, 2016. To register for the portal, an email address is required.

You will need to contact your medical provider’s office for any technical issues. But before you do, please check that you’re attempting to access your patient portal from a desktop computer using a Chrome, Microsoft Edge, Mozilla Firefox, or Safari browser.
