The patient portal is a website that allows you a convenient and secure way to view portions of your medical records and communicate with your provider’s health care team. Within your portal, you will be able to do the following:
At your next visit, tell the staff you would like to have a portal. The staff will request your email address, and may have you complete a consent form. Within 30 minutes, you will receive an invitation with a link to enroll. Just click on the link and follow the instructions to complete your enrollment.
Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a portal with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age.
Some of the information in your portal is provided through your medical health record and is viewable by you and authorized staff of Dignity Health facilities. If you would like someone to have access to your records to help manage your care, you can sign a release to authorize a trusted relative to be set up as a proxy.
The enrollment link expires 90 days from the day they are issued. If you are not able to find your invitation or it expired, you may request a new invitation from the staff at the facility that assist with your medical records.
To access you health records, communicate with your doctor, and more log into My Portal Patient Portal. If you do not have a My Portal account, please provide your email address to the office staff when you check in to your next appointment!
Remember, for a true medical emergency do not wait for a return call from the office. Instead, call 9-1-1 to obtain immediate access to local emergency medical services.
Some insurance companies require prior authorization and approval before your test can be scheduled. Your doctor will submit orders for any required tests; our Referrals and Authorization team will send required documentation to obtain authorization from your insurance company prior to scheduling.
Simply follow the steps in the “Action needed: Your Dignity Health records” email message you received.
During your clinic or hospital visit, you will receive an email invitation to create your account. Just click on the link and follow the instructions to complete your enrollment. If you do not receive an invitation please contact your clinic or hospital to request a new invitation.
