If you are unable to keep your appointment, please call the office as soon as possible to cancel your appointment.
If you have an emergency, such as chest pain, severe shortness of breath, severe headache, or bleeding, call 911 or proceed directly to the nearest hospital emergency room . You can always call the office with any questions about acute medical problems after hours and our answering system will page the physician on call.
Referrals. You should call the office at least three (3) days before the appointment to give us the information needed to complete the referral.

New patients are asked to arrive 30 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results, photo ID and your insurance card to your appointment.
Please contact your pharmacy for refill requests. Your pharmacy will fax us the request and your refills will be processed within 72 hours. You may also request refills on the patient portal.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards. (Visa, Master Card, American Express, Discover.)
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Mandatory screening and masking at all RDC facilities beginning Monday, August 2nd. As part of the OSHA COVID-19 Healthcare Emergency Temporary Standard (ETS), RDC will be implementing mandatory screening and masking for individuals entering any of our facilities. This is an OSHA mandate for healthcare facilities.
