How to get a Denver Health activation code?from mychart.denverhealth.org

If you do not have or know your social security number (SSN), please print and complete the form linked below to request your activation code. Submit the form via fax, e-mail or direct mail to one of the addresses in the upper left corner of the form. Once Denver Health receives the form, requesters will be notified within 4 business days.

If you encounter an activation issue and have your Medical Record Number (MRN) available, please contact MyChart Support at 303-602-4380. If you do not know your MRN, please call Health Information Management at 303-602-8000.

We need some information from you in order to create a MyChart account for you. Once submitted, it could take up to four business days for processing and verification. Once approved, you will receive an email or a letter with your activation code and instructions on how to activate your account. If you have any questions, please contact your clinic.

A third-party verification system confirms your identity by checking responses that you provide against their database of information. None of this information is stored at your clinic.

MyChart offers secure online access to portions of your child’s medical record. It enables you to easily and securely manage and receive information about your child’s health.

Once you’re registered, you can sign in to MyChart from your home computer at MyChart.childrenswi.org. You also can download the MyChart app free from your phone’s app store by searching “MyChart.” Select Children’s Wisconsin from the list to sign in to your account.

Once you have the activation code, you have 60 days to go to mychart.childrenswi.org and activate your account. We have staff at the Family Resource Center who also can help you create your account.
