A summary of your inpatient stay can be found on our Patient Portal. If you do not currently have a login, you will need your discharge paperwork to sign in. Records not included in the portal will need to be pulled from our Medical Records Department.

If the patient is under the age of 14, physically or mentally incapacitated or deceased a legally designated representative may complete the necessary forms on behalf of the patient and submit the request.
