To Create Your Account

To access the patient portal, a Dayton Gastroenterology staff member must have already provided you with a unique token ID number, which is used to initially create the account. Once you have a security token ID number, click on the green Patient Portal button and follow the steps to log into Dayton Gastro’s NextGen Patient Portal.

What to Know Before Your First Visit

The eye is one of the most complex parts of the body. Our eye care experts will conduct a thorough examination in order to properly diagnose any eye conditions or diseases and create a personalized treatment plan. You should plan to spend up to 2 hours at our office for your first visit or for any type of speciality exam.

The eye doctors at CVP Physicians Dayton provide medical coverage 24 hours a day for our established patients. In the event of an emergency after regular hours, please call (937) 320-2020 and your message will be forwarded to our physician-on-call.

Who is Dr. Dayton?

Jared Dayton is board certified in Family Medicine. He got his degree from the University of Health Sciences College of Osteopathic Medicine in Kansas City Missouri. He treats patients of all ages, from 1 day to 100yrs old. Dr Dayton knows that how we take care of our bodies now will affect how well we live in the next 10-30 years, emphasizing control of diabetes and cardiovascular risks. Dr Dayton also does: well male and female exams, well child checks, sports physicals, joint injections, weight loss, toenail removal and others. He has been married for 23+ years and has 4 sons, enjoys being outdoors – especially biking/hiking/camping, and just spending time with family.

How many kids does Alane have?

Alane has two children, ages 22 and 13, she lives in East Mesa with her husband who is a police officer and their daughter. When Alane isn’t at work you can find her spending time with her family, traveling, working-out, boating, hiking, or camping.

Who is Mallow Dayton?

Mallow Dayton is an emotional support specialist. He has provided comfort and love for all of Dayton Family Medicine’s patients for ten years. Mallow lives with a family of six which has given him vast experience in the provision of care and gentle affection. He is well experienced with providing the utmost comfort to patients of all ages with varying illnesses and concerns. Mallow enjoys office treats and being petted. Beyond this, he also serves as Lizard Patrol in the office, providing a lizard-free stay for all. An undeniable necessity to the office, Mallow Dayton looks forward to many more years with Dayton Family Medicine.

Where is Alane Larison?

Alane Larison is board certified as a Family Nurse Practitioner. She graduated from Ohio University and specializes in orthopedic, joint injection, women’s health, men’s wellness, well-child exams, nutrition, and cosmetic Botox. She tries to engage with each patient and their family to get a better understanding of their health-care needs. Alane has two children, ages 22 and 13, she lives in East Mesa with her husband who is a police officer and their daughter. When Alane isn’t at work you can find her spending time with her family, traveling, working-out, boating, hiking, or camping.
