What is the number to call Davita?

Where is my account number on my bill?

A: Your account number will start with a letter and is located on the bottom left-hand corner of your bill, followed by 6-7 numbers. The claim number and service dates will be in the middle section of the statement.

What is EOB insurance?

It is a document that you should use to determine how much you can expect to pay your provider. You should always match the provider’s bill with the EOB prior to making a payment to the provider.

When is Medicare enrollment?

For our Medicare patients. The Medicare Annual Enrollment Period takes place from October 15-December 7 each year. This is a time when you should review your Medicare plan to make sure it will still meet your needs in the coming year.
