Patients who have received care at Davis Regional Medical Center may request copies of their medical record/health information by completing an Authorization for Release, Use and Disclosure of Health Records (Autorización para usar y divulgarInformación de salud protegida) form.. Current Visitor Guidelines

In response to the increased presence of COVID-19 in our community, our hospital has implemented a restricted Patient Visitor Policy.

Visitation Specifics

We are allowing patients one designated support person (18 years and older with no COVID-19 symptoms) for all nursing care units and outpatient surgeries. Designated person will remain the same during hospital stay or visit, e.g. no switching of designees at this time.
