Patient Information

In order to keep the physicians on schedule it would be appreciated if you would complete the new patient forms prior to coming in for your appointment. When you arrive to your appointment the receptionist will ask for your paperwork and to make a copy of your insurance card (s) and drivers license.

Insurance plans

Our practice accepts most major insurance plans, as well as, Medicare. We also accept Amerigroup, Parkland, and Traditional Medicaid for children only.

What to expect at your first visit

When you arrive for your appointment, you will be greeted at our front desk by one of our administrative staff members who will review your patient paperwork as well as ask for your insurance card and driver's license to make copies of.

Additional information for the visit

Please read carefully the Patient Instruction/Consent form for Allergy Skin Testing. This will go over specifically what is involved with the skin testing and which medications that you or your child will need to stop 1 week prior to your scheduled appointment.

What time does the allergy shot room open?

Patients will still be required to wear a mask and social distance while in our shot room. Allergy Shot Room Hours: 8:00 am to 5pm (M-F) Except Tuesdays 8:00 am -7pm. Saturdays- Refer to shot card or dfwallergy.com for Saturday Hrs.

What are the symptoms of allergies in spring?

Spring allergies will be here soon. Symptoms include sneezing, stuffy or runny nose, itchy nose, eyes or roof of the mouth. Many people with untreated allergy symptoms are not aware of how much better they can feel once their symptoms are properly diagnosed and managed.

How to reduce pollen exposure?

Keeping your house and car windows closed, and limiting your outdoor activities on high pollen count days will help to reduce your exposure. If you are participating in outdoor activities, try to change your clothes and/or shower immediately after the activity to wash the pollens away.
