New Patients

For those who have never been to Boulder Community Health for their health care needs, we now offer online scheduling to create an appointment with one of our physicians or services.

TO CREATE YOUR ACCOUNT

To access the patient portal, a Desert Ridge Family Physicians staff member must provide you with a security token, which is used to initially create the account. Once you have a security token, click on the Initial Enrollment button.
