How do I enroll for the patient portal?

If you are currently a patient and have provided your email address, you can now enroll for the patient portal. Please click Login Request to begin. If you have a medical emergency, Dial 911.

Please print the Financial Policy Acknowledgment form, complete it and bring it to your appointment. Please review the HIPAA Policy. Then, please print the HIPAA Acknowledgment form, complete it and bring it to your appointment.

Have your primary care physician and/or your referring physician fax your records to (480) 218-4353.

Send us your referral

If your insurance plan requires a referral, please have your referring physician fax it to (480) 325-1390. If they already provided you with a paper referral, please bring the referral to your appointment. If you are arriving for testing only, please bring the physician order to your appointment.
