Be sure to include the correct spelling of their first and last names as well as their telephone, fax numbers and addresses. We also encourage patients to write down a list of questions they have for their surgeon, starting with the most important questions first.

How to Gain Access

CRS is available for all Maryland hospital users, MDPCP practice or CTO participants, health care organizations with ENS panels, and Maryland state and local health department staff.
