How do Crozer health patient portals work?

On November 2, 2021, Crozer Health will launch a consolidated patient portal, called HealthLink, for both inpatient and outpatient care. This technology allows us to have faster and more convenient access to our patients’ records wherever they go in the Crozer Health system and provide them with more efficient and effective care.

Patients scheduled for procedures at either of these locations are asked to complete their medical history online using One Medical Passport well in advance of their procedure.

How long does it take to complete a medical history?

Completing your medical history online using One Medical Passport is easy, and for most patients, takes less than 30 minutes.

Can I print my medical history online?

After you complete your medical history online, you will be able to print out a copy of your medical history and keep it for your records. In addition, you will be able to access your medical history online to review or update it.
