Crouse Medical Practice’s Patient Portal enables you communicate with your care team, renew prescriptions, and access your healthcare information securely and conveniently from your computer, tablet or phone from anywhere there is internet connectivity. Please, NEVER use patient portal for medical emergencies or any urgent clinical matter.
After setting up your account with a username and password, you or anyone else you designate, may view certain medical information related to your stay at Crouse. Upon discharge from our facility, you are able to view portions of your medical record, such as test results, immunizations, care plans and discharge instructions.
At Crouse Medical Practice, we are focused on your overall health and wellness. Our providers take time to get to know every aspect of your healthcare needs and build a trusting relationship with you. For your convenience you may print and complete the applicable Patient Forms and bring them to your visit.
NOTICE: Crouse Hospital’s MyHealthChart is only for current or any previous Emergency or Inpatient Crouse patients since March 1, 2014. If you have not been a patient with us, we will not be able to provision a MyHealthChart for you. Your “My Health Chart” patient account was created at the time of your discharge.
To submit the completed form, mail to Crouse Hospital, 736 Irving Ave., Syracuse, NY 13210, Attention: Health Information Management or fax to 315-470-5609.
If you wish to obtain a copy of your hospital medical record, please either fill out a request form or simply, on a sheet of paper, state your name, date of birth, what you would like to have copied and the address where the copies should be mailed.
