With our secure online patient portal, you can request appointments, referrals and prescription refills, and communicate with your care provider about routine (non-emergency) issues. We accept select Medicaid, Medicare, and private commercial and Affordable Care Act insurance plans. If you do not have insurance, our Outreach & Enrollment Specialists will help you sign up for Medicaid or a subsidized plan. Crossroad provides a sliding fee scale with a minimum co-pay of $15 for patients who do not qualify for one of these plans.
