When you sign-up for Health Record Home, you will be able to: 1 View and share your personal health record (PHR) related to your hospital stays. 2 Have electronic access to an updated list of medications, diagnoses, allergies, lab test results, patient history and other healthcare information related to your inpatient hospital visit. 3 Access your hospital discharge instructions for better after-discharge care. 4 Manage family members’ health records once you are authorized.

What is your username on Portal?

Your username will always be your email address. Each time you enter your Portal account, you also will need to enter your password and answer your security question for extra security.

Through your Cottage Health MyChart account, you will have the tools you need to better manage your health care online at your convenience.

Security is Our Top Priority

Your MyChart username and password ensure that your information is safe and confidential. If you forget your password, you are able to reset by clicking on the “Forgot Password?” link on the MyChart login screen. After answering security questions, you will be able to select a new password. You should never share your MyChart password.

View and Pay Bills Online

With MyChart you can view your bills and use your credit card to make payments online.

Manage Multiple MyChart Accounts Through MyChart Central

If you have another MyChart account with another health care provider, you may use MyChart Central to link your accounts and have a single login page for ease of use.
