Why should I use this web-based patient portal?

Through your portal, you can quickly and conveniently place service requests and schedule rent payments – anywhere, anytime.

How do I Activate my Patient Portal account?

You will be prompted to fill out some basic personal information, as well as create a login name and password and enter your name and date of birth exactly as it appears on the portal letter you receive during your visit. You will be able to make changes to your personal information once your portal account has been activated.

Before you submit an application make sure you are CITI certified. First, download and read these enrollment Instructions. You will not be approved to conduct any research at SUNY Cortland until you have the certification.
