For patients 18 years and older, please complete the Follow My Health Patient Portal Invitation Proxy Request Form and return it via fax to (919) 481-3952, mail it to our office at 97 Cornerstone Drive, Cary, NC 27519 or deliver it to our office. Please note: The 18yr old patient must be the person to fill out the form and sign it.
