What forms do I need to sign up for Coram?

Help with paperwork, including forms you’ll sign in order to receive care from Coram, like: Assignment of Benefits form that allows Coram to bill your insurance. Plus, it says you agree to the care prescribed by your doctor. Financial Arrangement Agreement form telling us how you’d like us to bill your out-of-pocket costs.

What is the number to call for a Coram refill?

If you receive an automated refill reminder call, the number on Caller ID will be 1-877-936-6874. Accepting this call will allow you to connect with a Coram refill representative, who will arrange monthly delivery of supplies to your home.

Does per diem include tube feeding?

The per diem does not include nursing visits or tube feeding formula which are billed and reimbursed separately. Track your delivery. Check the delivery status of your supplies and/or medications by entering your UPS tracking number here .

Can you return Coram prescriptions?

Once products have been delivered, they can’t be returned for credit, even if the prescription changed. State pharmacy laws do not allow Coram to accept any returns of unused supplies and products. It is important to share accurate information about your inventory of supplies and medication before deliveries are due.
