What is the COPC patient concierge form for?

This form is used for COPC's patient concierge team to help you find a new COPC doctor. It is not intended to request an appointment for a COVID-19 vaccination.

How do I log in to the patient portal?

Your doctor's office will provide you with a username and password to give you Patient Portal access. You can also get access by calling our Customer Service Department at (614) 423-6916 or toll-free at (844) 245-5538. The first time you log in, you will need to review the consent forms and click on next, then check the box, and click on agree.

How do I pick up a prescription at COPC Westerville?

Pay a fee for the patient's physician to review their medical records and re-write the prescription - which will then need to be picked up at the office. Antibiotic and narcotic prescriptions will not be called in to a pharmacy without an office visit Closed major holidays. Select a map service to get directions to COPC Westerville.

How do I add a dependent to my Patient Portal account?

Use the searchbox at the top of the screen and choose how to find the account (by provider name, practice name, or practice code/phone number.) Then select the “Search” button and choose the appropriate provider. Enter the Patient Portal username and password for the dependent you wish to add.

Prescription refills will be handled at office visits, and enough medicine will be prescribed to last at least until the patient's next appointment.
