MyLVHN patient portal

Beginning Sept. 1, 2020, Coordinated Heath Scranton Orthopedics patients have transitioned to MyLVHN, Lehigh Valley Health Network’s patient portal. This change has occurred as part of Coordinated Health’s transition to the Epic electronic health record system.

Orthopedics Walk-in Injury Center

No one ever plans for an injury. That is why we created Coordinated Health Scranton Orthopedics Walk-in Injury Center. You can just walk in, no appointment necessary, and receive treatment for any bone, joint or muscle issues.

Prescription refills

Prescription refills can be requested Monday – Thursday, 8 a.m.–5 p.m. and Friday, 8 a.m.–noon. at 570-307-1767 . Please allow 48 hours for your pharmacy to receive your prescription.

Washington Apple Health (Medicaid)

People without health insurance may be able to get free or low-cost health coverage from the Apple Health (Medicaid) program. This program covers medical and dental office visits, prescription drugs, eye exams, glasses and much more.

The Apple Health Core Connections program is a new managed care program that will provide coordinated health care services for children and youth in foster care or adoption support, and young adult alumni of the foster care program.

What is clinical event notification?

The clinical event notification allows clinicians and care managers to receive real-time alerts for hospitalizations, discharges, and other important events that occur regarding their patients. This feature enables the care team to better coordinate the patients’ care and reduce avoidable utilization. Additionally, clinical event notification provides automatic routing of discharge summaries from hospitals that is meaningful use (MU) and transition of care (TOC) compliant.

What is referral management?

Referral management services provides the ability to build preferred networks between physicians and facilities, and is highly customization based on cost and quality. This tool enables better care coordination through increased efficiency of automated pre-authorizations, automated reminders, and closed loop referrals, all while integrated into the native EHR workflow.

"Approved Hospitals" have been determined by the California Children's Services (CCS) program to meet the requirements in order to render services to a CCS applicant or client.

Paneled Non Provider Master File (PMF) Providers

"Paneled Non-PMF Providers" are required to be paneled, but not required to obtain a Medi-Cal provider number.

Paneled Providers

"Paneled Providers" have been determined by the CCS program to meet the advanced education, training, and/or experience requirements for his/her provider type in order to render services to a CCS applicant or client.
