Access to your child's electronic health record varies by age: Under 14: Parents have full access to the child's MyChart account. Ages 14-18: In order to provide confidentiality for our patients, parents have limited access. Parents can continue to message the care team and see test results.
MyChart is an online medical records system used by many major health care systems including Children’s Health℠. MyChart provides a comprehensive digital dashboard of your child’s health care needs and can be accessed in English and Spanish, both online and via the free smartphone app.
Once you download the MyChart mobile app, choose Children’s Health℠ from the Provider List. Scroll down and click the “Request Online link.”. Complete and submit the form, you’ll hear from us in one business day.
Pre-check in is a feature available through MyChart that allows you to complete the check-in process before you arrive for your child’s appointment. You will receive a MyChart notification that pre-check in is available 24 hours prior to your appointment.
Once you have the activation code, you have 60 days to go to mychart.childrenswi.org and activate your account. We have staff at the Family Resource Center who also can help you create your account.
MyChart offers secure online access to portions of your child’s medical record. It enables you to easily and securely manage and receive information about your child’s health.

MYchart Bedside (when your child is staying in the hospital): Log in from the tablet provided to you or your personal tablet. MYchart Bedside uses a temporary account that you can activate and turn into a full MYchart account if you do not have one.
Certain notes may not be available in MYchart and MYchart Bedside due to state or federal regulations that limit sharing of such notes with minors’ parents or legal guardians.

This process shouldn’t take long; it just depends on how busy the clinic is. Clinic staff will give you an activation code and instructions. You have 45 days to go online and complete your account activation. It shouldn’t take long, but the account activation has security measures built in to keep your child’s health information safe and secure.
Parents and primary caregivers can sign up to access MyChart for their children under the age of 13. For patients 13 to 17, parents or primary caregivers can access their child’s account as long as the child signs the MyChart assent agreement. (This is necessary to make sure adolescents understand that some aspects of their health information will be available to their parents or caregivers.) In addition, patients who are 13-17 may sign up for their own MyChart account as long as at least one parent or guardian has an account as well. If a child 13 to 17 declines to sign the assent agreement, neither the patient nor the parents or primary caregivers can access the medical record using MyChart. (Exceptions to this policy are made for children with diminished capacity.) Patients over 18 may sign up for their own MyChart account. Adult patients can also grant temporary proxy access to other family members, such as parents, siblings or spouses. For patients over 18 who are of diminished capacity, proof of continued legal guardianship is required.
If your child is 12 years old or younger or you are 18 years old or older, you may now go online to request a MyChart account. Simply select the form that best describes you. A photo ID is required to sign up online, so look for the secure upload to submit your proof of identification. Please allow 7 business days when requesting a MyChart account online.
If your child is between the ages of 13 and 17, you must come to a Cincinnati Children’s clinic to receive an activation code to sign the necessary paperwork. A MyChart agreement and assent form is required at 13 because state and federal laws recognize additional medical privacy rights for adolescents. These forms must be completed in person. We want to keep your children – and their information – safe.
When the child turns 18: A parent or guardian can no longer access a patient’s MyChart account after the child turns 18 unless the parent or guardian can provide Cincinnati Children’s with appropriate proof of ongoing legal guardianship (as in cases of diminished capacity). Adult patients can also grant temporary proxy access to other family members, such as parents, siblings or spouses.
You can only message providers you or your child has seen in the past three years, or providers that you or your child are scheduled to see in the upcoming year.
MyChart accounts for Cinci nnati Children’s patients will not expire. Parent or guardian proxy access will expire according to these rules:
