How do I sign up for my patient connect?

In the future, the two portals will be combined but for now, patients need to log in to separate portals based on where they receive their care. If you're a patient at Concord Hospital-Laconia or Concord Hospital - Franklin, and you want to access the patient portal for these two hospitals, visit concordhospital-laconia.org.
