Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (713) 791-9100 during regular business hours.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

We are happy to help with refill requests during regular office hours, so please call us at (713) 791-9100 to let us know what you need.

After-Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (713) 791-9100 and our answering service will assist you.

Office Hours

Our providers see patients by appointment only, 8:00 A. M. – 5:00 P. M. – Monday through Friday. We have multiple providers at each location every day for your health care needs.#N#New Patients are welcomed for Preventative Care or Problem appointments. To meet the needs of our existing patients, we have same day / urgent appointment availability.

Telephone Calls

Our helpful employees answer the telephone from 8:00 A. M. – 4:45 P. M. Monday through Friday. If you have a clinical question or concern, our clinical staff is available during business hours and can answer your questions or schedule an urgent appointment. After business hours, all calls go to our answering service.

Insurance

There are many different insurance plans and we have found that many Insurance Plans have different types of coverage. We recommend that prior to your appointment; you call your insurance company and verify that we are participating physicians with them.
