What is the community hospital patient portal?

This interactive web portal empowers all Community Hospital patients and their families to take a more active role in their care by providing easy, secure access to health information and online communication with Patient Portal staff. All of the information in the Patient Portal comes from your Community Hospital Electronic Health Record.

What can’t I See in the patient portal?

Sensitive subject matters such as pregnancy, sexually transmitted diseases, and certain other conditions are not appropriate for the Patient Portal. Parents will be able to view their minor children’s records up to their child’s 12th birthday, provided they are legally authorized.

Who can enroll in the patient portal?

Patients or their legal guardians can enroll in the Patient Portal if they agree to follow the terms and conditions spelled out on the enrollment form and if they have a valid e-mail address. Patients and/or legal guardians accessing their records through the Patient Portal may view, print, and/or forward their medical record information.

Where can I find a secure patient portal?

Auburn Community Hospital, Finger Lakes Medical Urgent Care Center and Urgent Care of Skaneateles offer a secure patient portal with easy access to your healthcare information from hospital visits and tests from your computer, tablet or phone.

I have been a patient of Dr. Schue for about 6 years, now. I started with him in Kokomo, IN and followed him, when he moved to Indianapolis, IN. I have very bad fibromyalgia and some arthritis. He is very attentive and answers any question I have. I have not seen a more caring doctor in many years.

What is the Patient Portal?

The Patient Portal is not intended to help diagnose a medical condition, provide any sort of treatment advice, or emergency help. If you are experiencing a medical emergency, you should be seen by the Emergency Department or you should call 911.

What is the phone number for HIM?

If you did not provide your e-mail address while being registered, please call HIM department, 304-487-7257, Monday-Friday. 8:00 a.m. – 4:30 p.m. and provide your e-mail address. Your e-mail address will be added to your account information. Once this information is added, you can self-enroll at your convenience.

Can a diagnosis be made after seeing a doctor?

Diagnosis can only be made and treatment provided after the patient has seen a doctor. Sensitive subject matters such as pregnancy, sexually transmitted diseases, and certain other conditions are not appropriate for the Patient Portal.
