What is the community hospital patient portal?

This interactive web portal empowers all Community Hospital patients and their families to take a more active role in their care by providing easy, secure access to health information and online communication with Patient Portal staff. All of the information in the Patient Portal comes from your Community Hospital Electronic Health Record.

What is Community Healthcare Network (CHN)?

Community Healthcare Network (CHN) is a federally-qualified community health center (FQHC) and participates in the federal medical professional liability program known as the Federal Tort Claims Act (FTCA). CHN and its staff are deemed a Public Health Service (PHS) Employee with respect to medical malpractice or other healthcare related claims.

How do I sign up for the patientportal?

Please send an email to: PatientPortal@chcrr.org How do I sign up? Be or become a patient at any one of our practices. You will need a portal activation code. (To obtain your portal activation code, call our Centralized Medical Records department at 802-671-5800, call your provider’s office, or stop by your provider’s office.)

Is there a patient portal for CHP users?

This patient portal is not a wellness portal for our CHP users. If you are a School District 51, Mesa County or Strive employee, please visit the employer groups tab above. Securely message your health care provider For questions regarding the Patient Portal, please call (970) 644-4002.

Log in to your Patient Portal account to use a credit or debit card to pay your medical bill and your payment will be applied directly to the balance on your Community Health account.
