The clinical use of oral appliances (OAs) for the treatment of snoring and obstructive sleep apnea (OSA) has markedly increased along with scientific research showing its effectiveness. Unfortunately, most dentists providing this therapy are not properly trained in sleep medicine nor are they associated with a comprehensive sleep center. Our dental sleep medicine facility is the only AADSM-accredited (American Academy of Dental Sleep Medicine) oral appliance program in the state of Colorado. We are fortunate to have on our team, Dr. Lindsey Deol, Boulder County’s only Diplomate of the American Board of Dental Sleep Medicine.

Medical Eye Evaluation Forms

In order to decrease the amount of time prior to your appointment, we recommend that you complete the new patient exam forms from this page and bring them with you to your exam.

Medical Records Release Forms

In order to release a patient’s medical records we need to have the following form filled out. The form must be signed by either the patient or their legal representative, electronic signatures are not accepted. The form can be submitted in officer or via fax.
