How do I get an FMH portal invite?

Must be listed on the patient's current HIPAA release to receive a portal invite. Please contact the patient’s care provider to update the appropriate HIPAA release forms. To send you a FMH invite, we must first verify your identity through a video call. Please list 2 days & times to best contact you for this verification call.

How do I request an invite to the myphc patient portal?

If you are an existing PHC patient but do not have a MyPHC Patient Portal account, please fill out the form below to request an invite to our portal. If you are not the patient, please leave your first and last name below.
