You can also get access by calling our Customer Service Department at (614) 423-6916 or toll-free at (844) 245-5538. The first time you log in, you will need to review the consent forms and click on next, then check the box, and click on agree.

When You Visit

When you come to our office you will be asked to bring a photo ID and your insurance card with any co-pay that you are responsible for.

Prescription Policies and Information

Prescription refills will be handled at office visits, and enough medicine will be prescribed to last at least until the patient's next appointment.
