Yes, you can download Patient Forms, print and complete. If the forms are complete when you arrive, then you will only need to be here 10-15 minutes early. If the forms are not completed, please arrive 30-45 minutes early.

Yes, if you are having a procedure done, you may not be able to drive.
Bring a detailed list of medications, past medical history, doctor’s notes, x-rays, and MRIs.
Use ice for 10-15 minutes to relieve pain and to keep the swelling down.
You would only use heat for 10-15 minutes when the injury is no longer acute. Heat helps to restore range of motion and decrease soft tissue stiffness.
