What do I need to bring for my first visit?

New patients must bring identification, your referral and insurance information. If under 18, a legal guardian must be present with you.

Can I fill out the medical forms prior to my visit?

Yes, you can download Patient Forms, print and complete. If the forms are complete when you arrive, then you will only need to be here 10-15 minutes early. If the forms are not completed, please arrive 30-45 minutes early.

Do I need to be early for my scheduled appointment time?

If you are a new patient and you completed the patient forms prior to your arrival, then you will only need to be here 10-15 minutes early. If you have not completed the forms, please arrive 30-45 minutes early.

Do I need to bring a companion with me?

Yes, if you are having a procedure done, you may not be able to drive.

What information should I bring for the doctor?

Bring a detailed list of medications, past medical history, doctor’s notes, x-rays, and MRIs.

When do I use ice on my injury?

Use ice for 10-15 minutes to relieve pain and to keep the swelling down.

When do I use heat on my injury?

You would only use heat for 10-15 minutes when the injury is no longer acute. Heat helps to restore range of motion and decrease soft tissue stiffness.
