Does CNY Fertility Center provide primary care services?

CNY Fertility Center does not provide primary care or associated wellness screening for general health management. It is each client’s personal responsibility to maintain routine primary care and gynecological evaluations including but not limited to pap smears and mammograms.

Who is the medical team for infertility?

While the medical side of your care will always be lead by a team of board certified reproductive endocrinologists and other fertility specialists, the emotions and stress that arise during infertility are very real and can be equally difficult to navigate.

How much down do you need to pay for fertility treatment?

Our in-house payment plans allow everyone to finance their fertility treatment with just 25% down. No credit check, no problems.

Do all fertility clinics accept everyone?

We believe everyone has the reproductive right to the fertility care they need. While many clinics restrict treatment options based off of age, BMI, and FSH/AMH levels, we accept everyone. True success is found only in helping everyone.

What is parental access in New York?

Parental Access - State laws concerning minors permit or require certain disclosure of protected health information to parents, guardians, and persons acting in a similar legal status. We will act consistently with the laws of New York and will make disclosures following such laws. CNY Fertility, PLLC will notify any patient over the age of 12 of a request to review his/her protected health information by a qualified person and reserves the right to deny the request should the minor patient object to the disclosure.

How to request a restriction on health information?

Right to Request Restrictions - You may ask us not to use or disclose (share) any part of your protected health information for treatment, payment or health care operations. Your request must be made in writing to our Privacy Officer. In your request, you must tell us: (1) what information you want restricted; (2) whether you want to restrict our use or disclosure, or both; (3) to whom you want the restriction to apply, for example, disclosures to your spouse; and (4) an expiration date. If we believe that the restriction is not in the best interests of either party, or that we cannot reasonably accommodate the request, we are not required to agree to your request. If the restriction is mutually agreed upon, we will not use or disclose (share) your protected health information in violation of that restriction, unless it is needed to provide emergency treatment. Pregnancy is not without risk; therefore, before the initiation of all Assisted Reproductive Technology (ART) cycles, clients are encouraged to be evaluated for general overall health. Chronic medical conditions such as diabetes, hypertension and asthma should be well controlled before attempting to conceive. Women who have a medical disorder, such as high blood pressure or diabetes, should talk with their primary care provider and/or Obstetrician/Gynecologist before attempting pregnancy. It is important that health problems are under control. Our physicians or a member of our clinical staff may suggest further testing such as a complete physical and a signed medical clearance from a primary care provider or cardiologist and an Obstetrician/Gynecologist as there are increased risks for women with a medical condition. Women over the age of 35 may also be at increased risk for medical complications during pregnancy. In addition, women planning to pursue fertility treatments should optimize their weight. Obesity has been associated with infertility, a reduced chance of success with an increase in the risk of miscarriage and preterm birth. Your primary care physician can help you determine your ideal weight and refer you to appropriate resources for weight management.

What is a PGM-1 report?

Your Pregmune® PGM-1 Reproductive Immunology Assessment Report is a comprehensive report that will assist your physician in diagnosing and treating your reproductive issues. It is advised to consult with your healthcare provider (gynecologist, reproductive endocrinologist or reproductive immunologist) to review the report.

Is every lab test interpreted by a member of the Pregmune medical team?

Yes. Every single lab test result is reviewed, validated and interpreted by a member of the Pregmune medical and research team.

Is Pregmune out of network?

Pregmune providers are out-of-network with all insurance companies. The patient can attempt to submit a claim for reimbursement for the Pregmune PGM-1 Report cost to their insurance carrier by presenting the statement from Pregmune, which will include all the information and service codes that are required by most insurance carriers in order to evaluate reimbursement eligibility and amount.
