We will act consistently with the laws of New York and will make disclosures following such laws. CNY Fertility, PLLC will notify any patient over the age of 12 of a request to review his/her protected health information by a qualified person and reserves the right to deny the request should the minor patient object to the disclosure.
Right to Request Restrictions - You may ask us not to use or disclose (share) any part of your protected health information for treatment, payment or health care operations. Your request must be made in writing to our Privacy Officer. Pregnancy is not without risk; therefore, before the initiation of all Assisted Reproductive Technology (ART) cycles, clients are encouraged to be evaluated for general overall health. Chronic medical conditions such as diabetes, hypertension and asthma should be well controlled before attempting to conceive. Women who have a medical disorder, such as high blood pressure or diabetes, should talk with their primary care provider and/or Obstetrician/Gynecologist before attempting pregnancy. It is important that health problems are under control. Our physicians or a member of our clinical staff may suggest further testing such as a complete physical and a signed medical clearance from a primary care provider or cardiologist and an Obstetrician/Gynecologist as there are increased risks for women with a medical condition. Women over the age of 35 may also be at increased risk for medical complications during pregnancy. In addition, women planning to pursue fertility treatments should optimize their weight.
