Welcome to the CHC Patient Portal, your medical home on the Web. With Patient Portal, you can connect with your doctor through a convenient, safe, and secure environment. Already Registered? Need to Register? WHAT IS A PATIENT PORTAL?
The patient portal is accessible on your desktop, laptop, tablet, and your smartphones. Tablets and smartphones do not require an application but automatically appear with a mobile friendly version of the patient portal. WHAT CAN I DO ON THE PATIENT PORTAL?
West County Health Centers is pleased to offer you our Patient Portal. The Patient Portal is a tool that will allow you to communicate with your provider and Care Team using secure e-mail, request and view appointments, and change your personal demographic information, view your medical history, and fill out questionnaires before your visit.
Check your email inbox for the confirmation message to begin the registration process. The message will be sent from 'donotreply@cchc.com'. Click the link provided to create a new user.
You will be asked to enter your name, date of birth, unique user name and password. Important: Make sure you check the box to the right to verify your information is correct. Click the "register" button at the bottom of the screen.
Choose three security questions with the answers, then click Submit. On the next page, click "Main Menu" to view your clinical records.
From the patient list, click on the name of the patient you are authorized to view. The patient's clinical visits will be listed chronologically. Next to each visit is a link containing the account number and the title "Summary of Care". Click on the link in the Summary of Care column to view the document.
The Patient Portal is for communicating NON-URGENT medical needs or concerns.
To sign up for the Patient Portal, please contact your medical office or ask your provider or Care Team during a future appointment.
